Job Description
The Family First Family Health Team (FHT) is looking for a committed and experienced Registered Dietitian to join our dedicated team of allied health providers and administrative personnel for a 12-14 month term.<br /><br />*Term (12-14 months)*<br /><br />*37.5 hours per week*<br /><br />*Primary Responsibilities*<br /><br />* Provide nutritional counselling in individual, family, and group settings;<br />* Conduct nutrition assessments;<br />* Promote behaviour change related to food choices, eating behaviour, and food preparation methods to optimize health;<br />* Monitor patient progress on a regular basis;<br />* Support patients in their efforts to make positive, healthy food choices;<br />* Collaborate with physician and other allied health professionals to incorporate a nutritional component in the management of chronic disease such as obesity, hypertension, diabetes, dyslipidemia;<br />* Support patients in their efforts to achieve individual nutrition goals;<br />* Implement strategies to promote health and prevent disease for both primary and secondary prevention;<br />* Initiate health education and other activities that assist, promote and support clients as they strive to achieve the highest level of health;<br />* Deliver health education programs for patients and communities;<br />* Develop learning resources for patients;<br />* Help patients identify and use health resources;<br />* Involve patients in decisions about their own health and care;<br />* Encourage patients to take action for their own health;<br />* Help educate students, peers, and other healthcare providers; and<br />* Act as a health advocate for patients.<br /><br />*Education, Training and Licensing Requirements*<br /><br />* Minimum of 3 years of recent continuous experience as a clinical dietitian in a primary care or hospital setting.<br />* Bachelor of Science in Nutrition (or similar) from a recognized school of nutrition accredited by the Partnership for Dietetics Education and Practice (PDEP) or, prior to 2015, Dietitians of Canada.<br />* Maintain licensure and registration with the College of Dietitians of Ontario; and<br />* Maintain current membership and evidence of adequate liability insurance through a professional association or another insurance carrier.<br /><br />*Salary*<br /><br />* $38.79 per hour based on experience.<br />* Eligible for benefits and Healthcare of Ontario pension plan (HOOPP).<br /><br />.<br /><br />Job Types: Full-time, Fixed term contract<br /><br />Pay: Up to $38.79 per hour<br /><br />Expected hours: 37.5 per week<br /><br />Work Location: In personBrowse Jobs in Canada by City
